Eligibility Criteria
Applicants must meet the following conditions:
- Must be a resident of Punjab
- Valid CNIC or B-Form (guardian CNIC for minors/mentally disabled)
- Disability Assessment Certificate from the Disability Assessment Board
- For hearing aids: ENT consultant recommendation from DHQ Hospital
- For wheelchairs/prosthetics: Orthopedic recommendation from DHQ Hospital
- Must be registered in the PWD Management System
- Applicant must not have received assistive devices previously from any government welfare scheme
